Prescription Assistance for Seniors

Improving access to existing resources for seniors

who cannot afford their medications.

Eligibility 

Criteria for participation:

*60 years of age or older.

*Ashtabula County resident.

*No third party prescription coverage (discount

cards are not considered insurance coverage.),

*Agree to the conditions of participation.

This program is designed to meet long-term

prescription drug needs (not immediate or short

term needs, for example - antibiotics).  Length

of coverage and eligibility criteria are determined

by the individual pharmaceutical companies.

A trained caseworker will meet with an applicant to:

*Help assess eligibility through the various patient

assistance programs.

*Aid in completion of the appropriate application

form or procedures.

*Assist with physician contact.

*Conduct follow-up.

All information will remain confidential.

What information is needed to make application?  Top of page

You will need to bring the following items with you:

*Proof of household income.

*Your prescription bottles and printout from pharmacy.

*Proof of age.

*Proof of residency.

*Health insurance cards.

*Name and address of your doctor(s).

*Other documentation may be required.

How long does the process take?  Top of page

Once the application forms or procedures are completed,

contact will be made with your physician.  Generally,

individuals can expect to receive their medication within

4-6 weeks (this varies by pharmaceutical company,

physician's office, and medication).

The Prescription Assistance for Seniors program

provides improves access to existing prescription

assistance programs to help meet the needs of

individuals who cannot afford their medications.

The Prescription Assistance for Seniors program

is funded through Senior Services Levy monies;

Ashtabula County Board of Commissioners,

and Fiscal Agent. Administered by the Ashtabula

County Department of Job and Family Services.
