Healthy Care Coverage for Ohio's Families

Healthy Start / Healthy Families offers free and low cost

healthy care coverage to families, children up to the age 19,

and pregnant women.

Simple application process:

*Short, two-page mail-in application.

*No need to apply in person.

Return the completed, signed, and dated application to the

Ashtabula County Department of Job & Family Services

along with the following items as they apply to your situation

Proof of:

*All income for past 30 days.

*Pregnancy.

*Health insurance.

*Alien Status, if NOT a U.S. citizen

*Delivery of baby

*Vision services

*Dental care.

*Substance abuse.

*Hearing test.

*Prenatal care.

*Mental health

Resources such as vehicles, bank accounts, and life

insurance are not counted.

Healthy Families (HF) Top of Page

Health coverage for the entire family with income at

or below 100% of the Federal Poverty Guidelines

(FPG).

*Must reapply every 6 months.

Healthy Start (HS) Top of Page

Health coverage for children up to age 19 and pregnant

women with a household income at or below 150% of the

FPG.

*Must reapply every 12 months - income changes may affect

eligibility status.

*Pregnant women eligible during entire pregnancy and for 60

days postpartum.

*Babies born to a mother receiving HS are eligible for free

health coverage for 1 year.

*Other health insurance does not effect eligibility.

Healthy Start Expansion (HSE) Top of Page

Health coverage for children up to age 19 with a household

income between 151% and 200% of the FPG.

*Guaranteed coverage for 12 months - regardless of income

changes.

*A small annual premium may be required -

$25/child, which a maximum of $75/family.

*Cannot have health insurance.  Health insurance is defined

as that which covers BOTH inpatient hospitalizations and

physician coverage- regardless of premiums, deductible,

and co-payments.
